Smart Card application submission date extended till November 15

Providing another opportunity to families missed to get their smart card issued under National Health Scheme and Mukhyamantri Swasthya Bima Yojana, the last date for the submission of application has been extended to November 15.
Eligible families can apply for the Smart Cart at the office of the Chief Medical and Health Officer, Old Nurses Hostel, Raipur.


They should come with either their Aadhaar card or ration card and contact Room No.25, ground floor of the office.
Application form is available free of cost. Those who have already applied for the Smart Card in camps held in 2016 need not to apply further. Similarly, the Smart Cards prepared after October 12 are still valid.


In Raipur, other centres where applications can be deposited are district hospital Kalibadi, community health centres Abhanpur/Arang/Tilda, Dharsiva. It is to be mentioned, upto
Rs 50,000 medical aid is being provided under the scheme.
Meanwhile, Congress staged a demonstration at the Directorate Ayush on Tuesday in protest against state government stand on the Smart Card date. The Congress alleged that the state government had last year cancelled thousands of applications. Earlier, November 7 was declared as last date for the submission of application.
